If you wish to proceed, please return a completed consent form, and our staff will be in touch with a booking link for your appointment.

Before your appointment, we will ask you to provide information about your health and complete clinical questionnaires, to help your doctor fully prepare.
04. Meet with your doctor
Attend your appointment via secure telehealth.
